Human error has been reported as main cause of the Black Hawk helicopter crash which resulted in the death of 3 Australian and one American commandos in Kandahar Province of Afghanistan in June last year.
Crew of three other Black Hawks immediately landed near the crash site in order to remove their fellow soldiers from the burning wreckage of the helicopter.
Australians have agreed with the American findings that there crash occurred due to the human error as helicopter suffered from no mechanical issues or enemy fire and no no distress calls were made by the helicopter crew.
